摘要

本文以1例合并高血压、腰痹（湿热痹阻证）的老年热淋（湿热下注证）患者为研究对象，基于“以通为用”理论，采用分阶段、分部位刮痧联合中药贴敷的中医外治方案，兼顾改善腰痹所致排尿异常，同时配合常规西医治疗、辨证施护与健康宣教。经阶段性干预后，患者尿路刺激症状、中医证候及实验室指标均显著改善，腰痹伴随症状缓解，未出现皮肤损伤、出血等不良事件。该中医外治护理方案安全可行，契合老年多病共存特殊人群的需求，可有效疏通下焦、清利湿热及疏通腰骶经络，促进膀胱气化功能恢复，具有较高的临床推广价值。

Abstract

This study focuses on an elderly patient with heat stranguria （damp-heat pouring downward pattern） complicated by hypertension and lumbar impediment （damp-heat impediment obstruction pattern）. Guided by the therapeutic principle of "treating obstruction with unblocking，" a staged and region-specific gua sha therapy combined with Chinese herbal application was adopted as an external TCM intervention regimen， which simultaneously addressed the voiding dysfunction secondary to lumbar impediment. The intervention was administered alongside routine Western medical treatment， pattern differentiation-based nursing， and health education. Following the staged intervention， the patient's urinary irritation symptoms， TCM pattern manifestations， and laboratory indicators were significantly improved， with concurrent alleviation of symptoms associated with lumbar impediment， and no adverse events such as skin injury or bleeding were observed. This TCM external nursing regimen is safe and feasible， well-suited to the special needs of elderly patients with multiple comorbidities， and demonstrates effective outcomes in unblocking the lower jiao， clearing damp-heat， dredging the meridians of the lumbosacral region， and promoting the restoration of bladder qi transformation， warranting broader clinical application.
