Integrated Traditional Chinese and Western Medicine Nursing Report on Phase I Cardiac Rehabilitation for a Patient with Acute Inferior Wall Myocardial Infarction (Killip Class IV) After PCI
To summarize the nursing experience of integrated traditional Chinese and Western medicine Phase I cardiac rehabilitation for a patient with acute inferior wall myocardial infarction (Killip class IV) following percutaneous coronary intervention (PCI).
Methods
2
Under intensive monitoring in the Coronary Care Unit (CCU), comprehensive assessments were performed, including hemodynamics, intra-aortic balloon pump (IABP)-related risks, exercise tolerance, psychological status, and Traditional Chinese Medicine (TCM) syndrome differentiation. A staged, extremely low-intensity exercise rehabilitation program was implemented, complemented by TCM syndrome differentiation-based nursing, emotional regulation, dietary instruction, auricular acupressure, TCM herbal hot compress therapy, and Baduanjin (Eight-Section Brocade) exercise training.
Results
2
The patient was successfully weaned from IABP on postoperative day 4 and discharged with improvement on day 10. No exercise-related adverse events occurred during the rehabilitation period. The levels of cardiac troponin I (cTnI), creatine kinase-MB (CK-MB), and N-terminal pro-B-type natriuretic peptide (NT-proBNP) showed an overall decline. The Self-Rating Anxiety Scale (SAS) score decreased from 57 to 42, and the Barthel Index increased from 40 to 60.
Conclusion
2
Phase I cardiac rehabilitation can be cautiously implemented in extremely high-risk patients with acute myocardial infarction (AMI) under rigorous assessment and monitoring. Integrated traditional Chinese and Western medicine nursing contributes to alleviating anxiety and promoting the recovery of cardiac function.
