摘要

本文总结1例老年2型糖尿病合并感音神经性聋伴耳鸣患者的中西医结合护理经验。针对患者激素治疗期间血糖波动、听力下降伴耳鸣、焦虑抑郁及睡眠障碍等多系统问题，实施血糖精准管理、中医特色外治护理、围治疗期护理、心理护理及健康教育等个体化干预措施。经19 d中西医结合护理，患者空腹血糖维持在7.6～11.2 mmol/L，餐后血糖峰值较入院时下降5.9 mmol/L，未发生低血糖；左耳0.25～4 kHz气导平均听阈提升5 dB；焦虑自评量表评分从63分降至42分，睡眠效率从65%提升至85%，治疗依从性达100%，患者满意度评分为98分。本案例可为老年2型糖尿病合并感音神经性聋伴耳鸣患者的中西医结合护理提供临床借鉴。

Abstract

This article summarizes the nursing experience of integrated traditional Chinese and Western medicine care for an elderly patient with type 2 diabetes mellitus complicated by sensorineural hearing loss and tinnitus. Aiming at addressing the multi-system problems encountered during hormone therapy， including blood glucose fluctuations， hearing loss accompanied by tinnitus， anxiety and depression， as well as sleep disturbance， individualized intervention measures were implemented， encompassing precise blood glucose management， traditional Chinese medicine characteristic external therapies， peri-treatment nursing care， psychological nursing， and health education. Following 19 days of integrated traditional Chinese and Western medicine nursing care， the patient's fasting blood glucose was maintained at 7.6-11.2 mmol/L， the peak postprandial blood glucose decreased by 5.9 mmol/L compared with that at admission， and no hypoglycemic episodes occurred； the air-conduction average hearing threshold of the left ear at 0.25-4 kHz was improved by 5 dB； the Self-Rating Anxiety Scale （SAS） score decreased from 63 to 42， sleep efficiency increased from 65% to 85%， treatment compliance reached 100%， and the patient satisfaction score was 98. This case can provide clinical reference for the integrated traditional Chinese and Western medicine nursing care of elderly patients with type 2 diabetes mellitus complicated by sensorineural hearing loss and tinnitus.
