Nursing Care Report of a Patient with Mixed-Type Lower Extremity Ulcer (Qi Stagnation and Blood Stasis Syndrome) Treated by Gradual Debridement Combined with Postural Management
Integrating the theoretical framework of traditional Chinese medicine (TCM)—encompassing theory, method, prescription, and medicinal substances, along with the concepts of Tao, method, technique, and instrument—this study summarizes the nursing practice of gradual debridement combined with dynamic postural management in an elderly patient with mixed-type lower extremity ulcer presenting as Qi deficiency and blood stasis syndrome, and explores its clinical application value.
Methods
2
A retrospective analysis was conducted on an 84-year-old patient with a refractory lower extremity ulcer persisting for four years, attributed to combined arteriovenous pathology. TCM pattern differentiation identified the syndrome as Qi deficiency with blood stasis, meridian obstruction, and loss of nourishment of the skin and muscles. Western medicine interventions included anti-infective therapy and circulation improvement as basic treatment. Adhering to the TCM principles of tonifying Qi, activating blood circulation, removing necrotic tissue, and promoting tissue regeneration, the nursing team employed gradual debridement and postural drainage as the guiding approach, syndrome-differentiated care as the methodological principle, and utilized techniques such as staged debridement and postural regulation, combined with prescriptions and instruments including Zhuhong ointment gauze strips and acupuncture tools, supplemented by health education. This integrated approach addressed both the superficial manifestations and the fundamental etiology to harmonize Qi and blood.
Results
2
After nine days of intervention, wound inflammation was effectively controlled, necrotic tissue was thoroughly removed, the wound surface reduced in size and became shallower, epithelial tissue migrated and proliferated, and granulation tissue appeared plump, fresh, and moist. The Visual Analog Scale (VAS) pain score decreased from 6 to 2, lower extremity stasis was alleviated, sleep quality improved, and no adverse nursing events occurred.
Conclusion
2
The nursing protocol constructed upon TCM thinking, with gradual debridement combined with postural management as its core, can improve local blood circulation, promote wound healing and tissue regeneration, and align with the pathogenic mechanism of ulcers in elderly patients with deficient constitution. This approach provides a valuable reference for the nursing care of similar refractory wounds.
