Nursing Report on the Treatment of a Case of Breast Abscess with Heat Accumulation Syndrome Using Manual Lactation Relief Combined with Abscess Aspiration and Drainage
This report presents a nursing case of acute mastitis (stomach heat abundance pattern, abscess formation stage) treated by manual lactation promotion combined with external application of Chinese herbal medicine. The patient was a lactating woman who presented with a painful mass in the right breast for over 10 days, with a Visual Analogue Scale (VAS) score of 7 points and mild anxiety. The Western medical diagnosis was acute mastitis with abscess formation, while the traditional Chinese medicine (TCM) pattern was identified as stomach heat abundance. Interventions included emotional and dietary regulation, manual lactation promotion to unblock the mammary ducts, acupressure at Danzhong and Rugen to regulate qi and blood, and external application of Rutong Powder (containing Taraxaci Herba and Phellodendri Cortex, among other herbs) to clear heat, remove toxin, resolve abscess, and dissipate nodules. Following treatment, the patient's pain was significantly relieved, with the VAS score decreasing to 1 point. Anxiety was alleviated, lactation became unobstructed, the breast texture changed from distended and hard to soft, and quality of life was improved. No adverse events occurred, and the patient reported high satisfaction. This case demonstrates that the integrated traditional Chinese and Western medicine protocol can effectively relieve pain and inflammation in patients with acute mastitis during the abscess formation stage, promote abscess absorption and recovery of lactation function, and improve anxiety status. The therapeutic effect is definite and worthy of clinical promotion.
