摘要

本文总结1例2型糖尿病合并足底破溃患者的个体化护理经验。患者左足底继发大面积水疱破溃，创面培养检出多重耐药路登葡萄球菌，以5A护理模式为核心，联合创面专科处置、中医特色护理及康复干预，结合4R危机管理落实风险管控。经43 d干预，患者创面愈合，血糖、足部感觉及心理状态改善，无不良并发症，随访足部未见新发损伤，预后良好。

Abstract

This article summarizes the individualized nursing experience in the care of a patient with type 2 diabetes mellitus complicated by plantar ulceration. The patient presented with a large ruptured bulla on the left plantar region， and wound culture identified multidrug-resistant Staphylococcus lugdunensis. Centering on the 5A nursing model， an integrated intervention was implemented that combined specialized wound management， Traditional Chinese Medicine （TCM）-characteristic nursing care， and rehabilitation training， with risk prevention and control ensured through the 4R crisis management framework. Following 43 days of intervention， the wound achieved complete healing， accompanied by improvements in glycemic control， foot sensation， and psychological status. No adverse complications occurred， and follow-up revealed no new-onset foot injury， indicating a favorable prognosis.
