Nursing Care for an Elderly Patient with Spleen-Deficiency Qi-Stagnation Constipation Based on the Theory of "Treating Both the Manifestation and Root Cause"
This paper summarizes the nursing experience of intervening in an elderly patient with spleen-deficiency qi-stagnation constipation based on the theory of "treating both the manifestation and root cause". The patient primarily presented with difficult defecation and abdominal distension. Grounded in holistic assessment and TCM pattern differentiation, the nursing care followed the principle of "regulating qi and unblocking the fu organs to address the manifestation, while fortifying the spleen and replenishing qi to address the root cause." The intervention comprised medicinal cupping therapy, auricular holographic scraping combined with auricular acupressure, complemented by pattern-based dietary guidance, exercise instruction, five-element music therapy, lifestyle regulation, and continuity of care. After an eight-day intervention, the patient's symptoms were relieved: the Patient Assessment of Constipation Symptoms (PAC-SYM) score decreased from 27 to 5; the TCM Symptom Score Scale for Gastrointestinal Diseases declined from 45 to 10; and the Patient Assessment of Constipation-Quality of Life (PAC-QOL) score decreased from 83 to 32. This case indicates that the comprehensive TCM nursing intervention based on the theory of "treating both the manifestation and root cause" can effectively improve defecation frequency, stool consistency, and constipation-related symptoms in elderly patients with spleen-deficiency qi-stagnation constipation, thereby enhancing their quality of life.
