摘要

本文报告1例72岁终末期心力衰竭（中医诊断：心水病，气虚血瘀证）患者心脏骤停后应用腹部提压心肺复苏技术（AACD-CPR）的中西医结合护理实践。患者因“间断胸痛憋气10年，加重3个月”入院，住院期间发生急性心肌梗死、心源性休克、呼吸衰竭，因存在肋骨骨折，故采用AACD-CPR替代传统胸外按压，复苏过程中患者曾两次恢复自主循环。护理措施包括循环支持、呼吸管理、水肿护理与皮肤护理等西医常规护理，以及情志调护、饮食调养、耳穴压丸与穴位按摩等中医特色护理。通过本案例的护理实践，本文总结AACD-CPR在“胸路不通”情况下的应用价值，探讨中西医结合护理在终末期心衰患者综合管理中的作用，为同类患者的救治提供参考。

Abstract

This paper reports the integrated traditional Chinese and Western medicine nursing practice for a 72-year-old patient with end-stage heart failure （TCM diagnosis： Xinshui disease， qi deficiency and blood stasis syndrome） who received abdominal active compression-decompression cardiopulmonary resuscitation （AACD-CPR） after cardiac arrest. The patient was admitted with a chief complaint of "intermittent chest pain and chest distress for 10 years， with symptom exacerbation for 3 months". During hospitalization， the patient developed acute myocardial infarction， cardiogenic shock and respiratory failure. Given the coexisting rib fractures， AACD-CPR was performed as an alternative to conventional external chest compressions， and the patient achieved return of spontaneous circulation twice during resuscitation. Nursing interventions included routine Western medical care covering circulatory support， respiratory management， edema care and skin care， as well as TCM-characteristic nursing measures including emotional regulation， dietary conditioning， auricular acupoint seed-pressing therapy and acupoint massage. Based on the nursing practice of this case， this paper summarizes the application value of AACD-CPR in scenarios where trans-thoracic CPR is contraindicated， discusses the role of integrated traditional Chinese and Western medicine nursing in the comprehensive management of patients with end-stage heart failure， and provides a reference for the treatment and care of similar patients.
