Nursing Report on the Application of Gua Sha Combined with Traditional Chinese Medicine Enema Based on "Holographic Theory" for Abdominal Pain in a Patient with Intestinal Obstruction
This case involved a patient with adhesive intestinal obstruction of the Qi stagnation and dampness obstruction type, who was admitted on April 13, 2026. On admission, the patient presented with cessation of flatus and defecation, nausea, vomiting, severe abdominal pain and distension, accompanied by electrolyte imbalance, hypoalbuminemia, and mild anxiety. On the basis of conventional Western medicine nursing care and guided by the Traditional Chinese Medicine (TCM) holographic theory, a characteristic integrated traditional Chinese and Western medicine nursing protocol was implemented, incorporating holographic Gua Sha on the second metacarpal bone and auricular regions combined with traditional Chinese medicine retention enema. The external intervention of holographic Gua Sha served to regulate visceral Qi dynamics and unblock the meridians, while the internal intervention of TCM enema aimed to dredge the intestinal fu-organs, promote Qi movement, and resolve dampness, achieving synergistic therapeutic effects through internal-external coordination. Following the entire course of nursing care based on syndrome differentiation, the patient's abdominal pain and distension were significantly relieved; bowel sounds, as well as flatus and defecation function, were progressively restored; anxiety was ameliorated; and all laboratory parameters returned to normal. The patient was successfully discharged on April 27, 2026. This protocol is safe, non-invasive, and highly feasible in clinical practice, and may provide a reference for the integrated traditional Chinese and Western medicine nursing care of patients with intestinal obstruction of the Qi stagnation and dampness obstruction type.
