摘要

本文回顾1例老年女性脑卒中合并焦虑抑郁患者的临床资料，总结基于老年综合评估（CGA）的个体化护理方案实施效果。根据CGA评估结果，基于多学科合作提出护理诊断，并采取针对性的个体化护理干预措施，包括健康宣教、心理护理、饮食护理、用药护理、康复护理等，旨在改善患者焦虑抑郁状态，提升患者睡眠质量和日常生活活动能力。

Abstract

This paper reviewed the clinical data of an elderly female patient with stroke complicated with anxiety and depression， and summarized the practice of individualized nursing plan based on comprehensive geriatric assessment （CGA）. According to the results of CGA， individualized nursing plan was made based on the nursing diagnosis and evaluation by a multidisciplinary team. Targeted interventions including health education， psychological nursing， diet nursing， medication nursing and rehabilitation nursing were carried out to relieve the anxiety and depression states， and improve patient’s sleep quality and activity of daily living.
