摘要

本文总结1例感染新型冠状病毒的急性肾衰竭患者，因心肌整体收缩减弱循环休克难以纠正行静脉-动脉体外肺膜氧合（V-A ECMO）联合连续肾脏替代疗法（CRRT）的护理体会。患者于2023年1月7日由医院急诊以急性肾衰竭、病毒性肺炎及血小板减少收治入院。护理要点为V-A ECMO时团队的配合、ECMO与CRRT的护理、血流动力学的监测、气管插管呼吸机辅助通气的护理、镇静的管理以及后期的营养支持、心理护理和各种并发症的观察及护理。患者3 d后恢复心功能，73 h后ECMO撤机，9 d后撤除呼吸机，10 d后停止CRRT，19 d后出院。

Abstract

This paper summarized the clinical data of patient with acute renal failure and COVID-19 pneumonia. The venoarterial extracorporeal membrane oxygenation （VA-ECMO） and continuous renal replacement therapy （CRRT） was carried out to alleviate the circulatory shock caused by decreased force of myocardial contraction. The patient was diagnosed with acute renal failure， viral pneumonia and thrombocytopenia. Key issues in nursing included teamwork in the V-A ECMO treatment， ECMO and CRRT nursing， hemodynamic monitoring， tracheal intubation and assisted ventilation， sedation management， nutrition support， psychological care and prevention of complications. Functional cardiac recovery was observed at 3 days after treatment. The patient used ECMO， ventilator and CRRT for 73 h， 9 days and 10 days respectively， and was discharged from hospital 19 days after admission，
