摘要
本文总结1例脑出血后偏瘫患者置入PICC导管后，合并III期压力性损伤伴金黄色葡萄球菌感染的护理体会。此病例肘部压力性损伤距离PICC穿刺点小于3 cm，护理团队召集多学科专家共同协作，采用集束化管理模式，评估穿刺周围皮肤，制定个性化的换药措施、糖尿病管理、营养管理，配合整体护理，同时在整个过程中不断评估、总结并持续改进护理措施。经过一系列个性化的护理，保障了患者的输液安全、纠正了患者营养状态、创面愈合良好，出院时拔除管PICC导管后，尖端培养未见微生物感染。
Abstract
This article summarized the nursing experience of a patient with hemiplegia after cerebral hemorrhage complicated with stage III pressure injury and Staphylococcus aureus infection after PICC catheter placement. In this case， the distance of elbow pressure injury to PICC puncture point was less than 3 cm. The nursing team called multidisciplinary experts to work together and adopted cluster management mode to evaluate the skin around the puncture， formulate personalized dressing change measures， diabetes management， nutrition management， and holistic nursing. At the same time， the nursing measures were continuously evaluated， summarized and improved during the whole process. After a series of personalized nursing， the infusion safety of patients was guaranteed， the nutritional status of patients was corrected， and the wound healed well. After the PICC catheter was removed at discharge， no microbial infection was found in the tip culture.
