摘要

本文总结1例伴有癌性伤口的老年共病患者的护理体会。根据患者临床资料，由责任护士汇报病例，医生分析病情，专科护士提出护理意见，医护联合针对患者症状进行讨论，采用线上+线下多学科协作。充分评估后，制定个性化护理方案，具体包括病情观察、癌性伤口护理、心理护理、营养管理、消毒隔离、非药物护理、出院指导。通过以专科护理为主导的医护一体多学科查房，将治疗与专科护理相结合，达到个性化、精细化、综合化的护理效果，为临床个体化护理提供依据。

Abstract

This article summarizes the nursing experience of an elderly patient with multimorbidity accompanied by cancer wound. According to clinical data， report the cases by a nurse， the doctor analysis condition， specialist nurses nursing opinions， discuss joint care for patients with symptoms， adopt multidisciplinary collaborative online and off-line. After fully assessed， make personalized nursing plan， including observation， carcinoma of wound care， psychological care， nutrition， management， disinfection isolation， non-drug nursing， and discharge guidance. Through dominated by specialized nursing care integrated multidisciplinary rounds， combined treatment and nurse， achieve personalized， refinement， integrated care effect， provide the basis for clinical individualized nursing care.
