摘要

压疮是由于局部组织长期受压，发生持续缺血、缺氧、营养不良而致组织溃烂坏死所致。本文总结1例肺炎患者合并2期压力性损伤伴发热的护理体会。患者右侧肩部2期压力性损伤，护理人员与科室压疮小组共同协作，采用集束化管理模式，评估穿刺周围皮肤，制定个性化的整体护理和皮肤护理策略，配合中医护理，同时在整个过程中不断评估、总结并持续改进护理措施。集束化护理措施有效保障了患者的置管安全，纠正了患者营养状态，促进创面愈合。

Abstract

Pressure injuries are caused by tissue ulceration and necrosis due to continuous ischemia， hypoxia and malnutrition. This paper summarizes the nursing experience of a pneumonia patient with stage 2 pressure injury with fever. Nursing staff and the pressure injury management team had adopted the clustered management mode to evaluate the skin condition， developed personalized strategies of holistic care and skin care， and carried out traditional Chinese medicine nursing techniques. At the same time， nurses had continuously evaluated， summarized and improved nursing measures during the whole process. Nursing interventions based on the cluster management are effective to ensure the safe use of catheter， correct the patient's nutritional status and improve the wound healing.
