摘要
总结1例高龄、长期卧床、依从性差，存在多种合并症，心功能评分极低的重症心力衰竭患者以问题为导向的护理模式的应用效果。本例患者病情迁延、复杂，随时可能发生生命危险，护理过程需考虑患者的特殊生理、心理状况及疾病的复杂性。患者存在呼吸困难、体液潴留、心功能严重下降、营养不良、心理问题、易发生药物副作用及潜在并发症等多方面护理问题。针对以上问题，制定个体化的护理计划，通过多学科协作医护人员共同探讨、综合治疗及重症监护护理等多种措施，患者病情逐渐平稳，感染得到控制，体温恢复正常，胸闷喘憋症状缓解，血压、血糖得到有效控制，住院期间未发生跌倒、坠床、压疮等不良事件。以问题为导向的护理模式可以更好的满足重症心力衰竭患者的需求，为患者提供更全面、有效的治疗效果及服务，经验值得临床借鉴。
Abstract
This article summarizes the application effect of a problem oriented nursing model in a severe heart failure patient with advanced age， long-term bed rest， poor compliance， multiple comorbidities， and extremely low cardiac function score. The patient's condition in this case is prolonged and complex， and there may be life-threatening situations at any time. The nursing process needs to consider the patient's special physiological and psychological conditions as well as the complexity of the disease. The patient has many nursing problems， such as dyspnea， fluid retention， serious decline of cardiac function， malnutrition， psychological problems， drug side effects and potential complications. In response to the above issues， an individualized nursing plan was developed. Through multidisciplinary collaboration among medical staff， comprehensive treatment， and intensive care nursing measures， the patient's condition gradually stabilized， infection was controlled， body temperature returned to normal， chest tightness and wheezing symptoms were relieved， blood pressure and blood sugar were effectively controlled， and no adverse events such as falls， bed falls， or pressure ulcers occurred during hospitalization. The problem oriented nursing model can better meet the needs of patients with severe heart failure， provide more comprehensive and effective treatment effects and services for patients， and the experience is worthy of clinical reference.
