摘要

本文总结1例金属弹珠致血气胸伴食管破裂合并失血性休克患者的护理体会。护理要点包括纠正失血、休克及呼吸功能障碍，维持有效氧合；围手术期的免疫营养支持，满足机体营养和代谢需要；胸腔闭式引流管的离心挤压方法，使堵管率降低，效果满意；量化功能锻炼避免笼统宣教，有效预防DVT形成及坠积性肺炎的发生。通过ICU医护团队紧密协作，精心治疗及护理，该患者于2022年12月2日转心胸外科继续治疗。

Abstract

This article summarizes the nursing experience of a patient with hemopneumothorax caused by metal marbles， accompanied by esophageal rupture and hemorrhagic shock. The nursing points include correcting blood loss， shock， and respiratory dysfunction， and maintaining effective oxygenation； Immune nutritional support during the perioperative period to meet the nutritional and metabolic needs of the body； The centrifugal compression method of closed thoracic drainage tube reduces the blockage rate and achieves satisfactory results； Quantitative functional exercise avoids general education and effectively prevents the formation of DVT and the occurrence of pneumonia caused by accumulation. Through close collaboration and meticulous treatment and care by the ICU medical team， the patient was transferred to the cardiothoracic surgery department for further treatment on December 2， 2022.
