摘要
本病例总结1例病毒性脑炎继发颅内感染的重症患者，患者因获得性衰弱需气管切开并持续呼吸机辅助通气，同时存在大量口腔及声门下分泌物及频发呃逆症状，严重影响呼吸功能锻炼，导致脱机时间延迟。针对患者特异性症状，通过护理评估制定个性化护理计划，明确首优、中优、次优护理诊断。护理措施包括：ICU专科技术：改良持续声门下吸引装置连接方式，及时清除分泌物以预防VAP；中医护理干预：联合耳穴压丸、穴位贴敷、针灸及艾灸治疗，有效缓解呃逆症状；康复强化：加强呼吸功能锻炼与肢体康复。最终患者成功脱机，带气管切开套管康复出院。
Abstract
This case report summarizes the management of a critically ill patient with viral encephalitis complicated by intracranial infection， who required tracheostomy and prolonged mechanical ventilation due to acquired weakness. The patient exhibited copious oropharyngeal and subglottic secretions along with frequent hiccups， significantly impairing respiratory rehabilitation and delaying weaning. A tailored nursing plan was developed based on comprehensive assessment， prioritizing primary， secondary， and tertiary nursing diagnoses. Key interventions included： ICU-specific techniques： Modified continuous subglottic suctioning to prevent ventilator-associated pneumonia （VAP）； Integrative Traditional Chinese Medicine （TCM） therapies： Auricular acupressure， acupoint application， acupuncture， and moxibustion to alleviate hiccups； Enhanced rehabilitation： Intensive respiratory and limb functional training.The patient achieved successful weaning and was discharged with a tracheostomy tube.
