摘要

本文总结1例肾病综合征合并重度阴囊水肿患者的护理经验。患者病情复杂、基础疾病多，阴囊水肿呈球状（16 cm×10 cm×12 cm），伴有明显胀痛及焦虑抑郁情绪。护理评估采用视觉模拟疼痛评分（VAS）、焦虑自评量表（SAS）及抑郁自评量表（SDS）进行量化评价；护理诊断聚焦于体液过多、皮肤完整性受损风险及焦虑状态；护理计划以快速缓解局部症状，维护皮肤完整，改善心理状态为目标。在常规治疗基础上，创新性采用玄明粉联合10%氯化钠注射液阴囊湿敷，辅以体位护理、饮食管理、情志护理及中医特色护理。干预后10 d，患者阴囊水肿完全消退，VAS评分由5分降至0分，SDS评分由52分降至32分，SAS评分由60分降至53分，疗效显著。本案例表明，中西医结合局部外治法在解决顽固性阴囊水肿方面具有起效快、操作简便、患者依从性高的优势，为临床处理类似疑难症状提供了可借鉴的护理思路。

Abstract

This article summarizes the nursing experience of a patient with nephrotic syndrome and severe scrotal edema （measuring 16 cm × 10 cm × 12 cm）， accompanied by significant distension pain， anxiety， and depression. Comprehensive assessments were conducted using the Visual Analog Scale （VAS） for pain， Self-Rating Anxiety Scale （SAS）， and Self-Rating Depression Scale （SDS）. Nursing diagnoses focused on fluid overload， risk of impaired skin integrity， and anxiety. The care plan aimed to rapidly alleviate local symptoms， preserve skin integrity， and improve psychological status. In addition to conventional therapy， an innovative intervention combining topical application of Xuanming Powder （a traditional Chinese herbal remedy） with 10% sodium chloride solution for scrotal compress was implemented， supplemented by postural care， dietary management， emotional support， and TCM-specific nursing techniques. After 10 days of intervention， the scrotal edema resolved completely， with VAS score decreasing from 5 to 0， SDS from 52 to 32， and SAS from 60 to 53. This case demonstrates that integrated TCM-Western topical therapy offers rapid efficacy， operational simplicity， and high patient compliance for refractory scrotal edema， providing a reference for managing similar complex conditions.
