摘要

回顾性分析1例长期服用氯氮平致肠梗阻的老年精神分裂症患者的中西医结合护理过程。患者70岁，因精神失常55年长期口服氯氮平，住院期间出现腹痛腹胀、停止排便48 h，中医辨证为阳明腑实、气滞血瘀证。予西医禁食水、胃肠减压、调整药物剂量等对症治疗，联合大承气汤保留灌肠、中药热奄包及穴位按摩等中医护理，同时加强精神症状监测与并发症预防。经14 d系统干预，肠梗阻完全缓解，每日自主排便1次，精神状态稳定。提示长期服用氯氮平患者需重点监测胃肠道反应，中西医结合护理可有效改善药源性肠梗阻症状。

Abstract

This paper retrospectively analyzed the implementation process of integrated traditional Chinese and Western medicine （TCM） nursing for an elderly patient with schizophrenia who developed intestinal obstruction secondary to long-term clozapine administration. The 70-year-old patient， who had received long-term oral clozapine treatment for a 55-year history of psychotic disorder， presented with abdominal pain， abdominal distension and an absence of defecation for 48 hours during hospitalization， and was differentiated as Yangming fu-organ excess syndrome with qi stagnation and blood stasis according to TCM syndrome differentiation. Symptomatic Western medical interventions including fasting （food and water deprivation）， gastrointestinal decompression， and clozapine dose adjustment were delivered， in combination with TCM nursing measures such as retention enema with Dachengqi Decoction， external application of Chinese herbal hot packs， and acupoint massage； intensified psychiatric symptom monitoring and proactive complication prevention were also implemented throughout the intervention course. After 14 days of systematic care， the patient’s intestinal obstruction was completely resolved， he achieved regular spontaneous defecation once daily， and his mental status remained stable. This case indicates that gastrointestinal adverse reactions should be prioritized in routine monitoring for patients on long-term clozapine therapy， and integrated TCM and Western medicine nursing can effectively ameliorate symptoms of drug-induced intestinal obstruction.
