摘要

本文报告1例因中药溻渍操作不规范导致浅Ⅱ度烫伤的患者诊疗经过。患者接受中药溻渍治疗时，因药液温度过高、接触时间过长，致左肩部出现浅Ⅱ度烫伤，创面面积约3.0 cm×2.5 cm。明确诊断后，采用复方黄柏液湿敷联合莫匹罗星软膏外用的中西医结合方案，2次/d，治疗第14天创面完全愈合，随访1个月无增生性瘢痕形成。该方案发挥了中西医结合治疗优势，复方黄柏液抗炎促愈，莫匹罗星软膏抗菌防感染。本案例提示，对存在感觉障碍的患者实施中药外治时，须以温度计精准测量药液温度，加强治疗过程中的主动监测，以防范医源性损伤发生。

Abstract

This article reports the diagnosis and treatment process of a patient with a superficial second-degree scald caused by non-standard Traditional Chinese Medicine （TCM） wet compress （Tazi） therapy. During the TCM wet compress treatment， the patient developed a superficial second-degree scald on the left shoulder due to excessively high temperature of the medicinal solution and prolonged contact time， with the wound area measuring approximately 3.0 cm × 2.5 cm. After a definitive diagnosis was made， an integrated Chinese and Western medicine regimen was adopted， consisting of wet compresses with Compound Huangbai Liquid （Fufang Huangbai Ye） combined with topical application of mupirocin ointment， administered twice daily. On the 14th day of treatment， the wound had healed completely， and no hypertrophic scar formation was observed during a one-month follow-up. This regimen leveraged the advantages of integrated Chinese and Western medicine， in which Compound Huangbai Liquid exerted anti-inflammatory and wound-healing-promoting effects， while mupirocin ointment provided antibacterial action to prevent infection. This case suggests that when implementing TCM external therapies in patients with sensory impairment， it is essential to accurately measure the temperature of the medicinal solution with a thermometer and to strengthen active monitoring during the treatment process in order to prevent iatrogenic injury.
