摘要

本文总结1例脱疽患者采用蚕食清创换药联合皮内针治疗的疼痛护理体会。在常规护理基础上，基于中医辨证施护原则，实施蚕食清创联合皮内针治疗，既能直接作用于疮面，又可调理脏腑气血。护理全程恪守脱疽“急则治标、缓则固本”及“祛邪不伤正、扶正不敛邪”的准则，有效缓解患者疼痛，提高生活质量。

Abstract

This paper summarizes the nursing experience of pain management in one patient with gangrene of the extremities treated by gradual debridement and dressing change combined with intradermal acupuncture. On the basis of conventional nursing care and guided by the TCM principle of pattern differentiation-based nursing， the combined therapy of gradual debridement and intradermal acupuncture was implemented， which could directly act on the wound surface while simultaneously regulating the qi and blood of the zang-fu organs. Throughout the nursing process， the therapeutic principles for gangrene of the extremities—namely， "treating the superficial manifestations in acute conditions and consolidating the fundamental constitution in chronic conditions" and "eliminating pathogenic factors without damaging healthy qi， while supporting healthy qi without retaining pathogenic factors"—were strictly adhered to， which effectively relieved the patient's pain and improved quality of life.
