摘要

本文总结1例急性下壁右室心梗IABP辅助PCI术后合并上消化道出血患者的分层中医护理实践。基于患者病情特点，护理团队实施严密循环监护、出血监测与管控、抗凝止血平衡管理、多管路精细化维护及并发症预防；针对卧床与出血双重禁忌，采用分层中医特色护理干预。经系统干预，患者出血停止、循环稳定且心功能逐步恢复，顺利康复出院。

Abstract

This article summarizes the stratified traditional Chinese medicine （TCM） nursing practice for a patient with acute inferior wall and right ventricular myocardial infarction complicated by upper gastrointestinal bleeding following intra-aortic balloon pump （IABP）-assisted emergency percutaneous coronary intervention （PCI）. Based on the patient's clinical characteristics， the nursing team implemented rigorous hemodynamic monitoring， bleeding surveillance and control， balanced management of anticoagulation and hemostasis， meticulous maintenance of multiple catheters， and prevention of complications. In view of the dual contraindications of bed rest and active bleeding， stratified TCM-specific nursing interventions were applied. Following systematic intervention， the patient's bleeding ceased， hemodynamic status stabilized， and cardiac function gradually recovered， leading to a successful recovery and discharge.
