摘要

本报告通过梳理1例老年女性失眠患者的护理过程，总结探讨面碗脐灸联合五音疗法干预肝郁脾虚型失眠的临床疗效。经中医证候评估，该患者辨证为肝郁脾虚证，因情志不畅导致失眠症状进一步加重，因此采用面碗脐灸联合五音疗法开展干预，以达到疏肝解郁、调理脾胃的作用。干预前后分别采用中医证候评分、失眠严重程度指数量表（ISI）、数字疼痛评分法（NRS）及汉密尔顿焦虑量表（HAMA）开展系统评估。结果显示，干预后患者的焦虑状态、睡眠形态均得到改善，头痛、腹胀等伴随症状也较干预前明显缓解。由此证实该联合疗法具备无创伤、低风险及整体调节的优势，可为失眠症的中医绿色诊疗提供新的可行思路。

Abstract

This report summarizes and discusses the clinical efficacy of surface-bowl moxibustion on the navel combined with five-element music therapy in the management of insomnia with the liver depression and spleen deficiency pattern， by reviewing the nursing process of an elderly female patient with insomnia. Following Traditional Chinese Medicine （TCM） syndrome assessment， the patient was diagnosed with the liver depression and spleen deficiency pattern， and emotional disturbance had further aggravated her insomnia symptoms. Accordingly， surface-bowl moxibustion on the navel combined with five-element music therapy was implemented to soothe the liver， relieve depression， and regulate the spleen and stomach. Systematic assessments were conducted before and after the intervention using the TCM Syndrome Score， the Insomnia Severity Index （ISI）， the Numeric Rating Scale （NRS） for pain， and the Hamilton Anxiety Rating Scale （HAMA）. The results demonstrated that the patient's anxiety status and sleep pattern were improved after the intervention， with accompanying symptoms such as headache and abdominal distension also notably alleviated compared with the pre-intervention status. The findings confirm that this combined therapy offers the advantages of being non-invasive， low-risk， and holistically regulatory， which may provide a new feasible approach to the green diagnosis and treatment of insomnia within the TCM framework.
