摘要

本文总结1例有室间隔缺损修补手术史的妊娠妇女剖宫产术围手术期静脉血栓栓塞症（VTE）的防治及护理经验。给予患者中西医结合治疗及护理，主要包括孕期进行血栓风险评估与跟踪；术前对患者及其家属进行静脉血栓相关知识的宣教提高其配合度；术后在常规护理的基础上行经穴推拿等中医特色护理技术，以调节泌乳、促进血液循环、缓解疼痛、预防尿潴留等，充分发挥中医特色护理技术在VTE防治等方面的特色优势。从孕中期门诊建卡到产后42 d随访，该患者孕16周、28周、35周、38周、产后1周及产后42 d这6个时间点的7项凝血功能指标变化差异具有统计学意义（

＜0.05），且未发生VTE等并发症，母婴结局良好。

Abstract

This paper summarized the prevention and nursing management of venous thromboembolism （VTE） in perioperative period of cesarean section in a pregnant woman with a past history of repair of ventricular septal defect. The patient received a combination treatment and nursing of Traditional Chinese and Western medicine. It mainly included risk assessment and tracking of thrombosis during pregnancy， preoperative education on venous thrombosis-related knowledge for patients and their families； After operation， on the basis of routine nursing， Traditional Chinese Medicine nursing techniques such as acupoint massage were used to regulate lactation， promote blood circulation， relieve pain， and prevent urinary retention. From the day of establishing outpatient mid-term pregnancy documentation to the 42

day of postpartum follow-up， there was a statistically significant changes in levels coagulation function indicators（ reported at 16

week， 28

week， 35

week， 38

week of pregnancy， 1 week postpartum， and 42 days postpartum，

0.05）， and no complications such as VTE occurred. The maternal and infant outcomes were good.
