摘要

冠心病二级预防是一种综合性的心血管疾病管理的医疗护理模式。我们对1例冠心病患者进行评估，患者存在知识不足、态度不重视、行为方式不健康、用药依从性差、缺乏有效的自我管理等不良习惯。我们从循证用药、运动评估及运动治疗、生活方式改变（包括戒烟）、营养康复等方面给予患者进行个性化的整体护理，帮助患者建立冠心病二级预防自我终身管理。通过冠心病二级预防护理，改善了患者的生活质量，降低了再住院的风险。

Abstract

Secondary prevention of coronary heart disease is a comprehensive medical nursing model for managing cardiovascular diseases. We evaluated a patient with coronary heart disease and found that the patient had poor habits such as insufficient knowledge， lack of attitude， unhealthy behavior， poor medication adherence， and lack of effective self-management. We provided personalized overall care to the patient such as evidence-based medication， exercise assessment and treatment， lifestyle changes （including smoking cessation）， and nutritional rehabilitation to help the patient to establish secondary prevention and lifelong self-management of coronary heart disease. Through secondary preventive care for coronary heart disease， the patient’s life quality was improved and the risk of readmission was reduced.
