摘要

急性呼吸窘迫综合征是重症肺炎患者常见的临床综合征，病情危重且死亡率高。急性呼吸窘迫综合征以呼吸窘迫和难治性低氧血症为临床特征，机械通气是主要的呼吸支持方法。经鼻高流量氧疗及无创正压通气可改善患者呼吸驱动，俯卧位通气可使肺内气体分布更加均一，两者均可以改急性呼吸窘迫综合征患者症状和预后。本文总结1例患重症肺炎者早期采取俯卧位通气联合经鼻高流量氧疗的护理体会，发现两者联合可更好地改善肺的呼吸驱动，对进一步完善重症肺炎患者呼吸支持的临床护理管理策略具有指导意义。

Abstract

Acute respiratory distress syndrome （ARDS） is a common clinical syndrome in patients with severe pneumonia， with critical illness and high mortality. ARDS is characterized by respiratory distress and refractory hypoxemia. Mechanical ventilation is the main method of respiratory support. Nasal high-flow oxygen therapy and noninvasive positive pressure ventilation can improve the respiratory drive of patients， and prone position ventilation can make the gas distribution in the lung more uniform. Both of them can improve the symptoms and prognosis of ARDS patients. This article analyzes the nursing experience of a patient with severe pneumonia who took prone position ventilation combined with high-flow nasal cannula oxygen therapy in the early stage. It is found that the combination of the two can better improve the respiratory drive of the lung， which has guiding significance for further improving the clinical nursing management strategy of respiratory support for patients with severe pneumonia.
