摘要

本文总结基于5E康复模式的中西医结合护理干预在1例脑梗死患者二级康复护理中的应用体会。5E康复模式以患者为中心，契合中医整体观，通过“鼓励、教育、运动、工作、评估”5个方面的有效介入，对患者肢体功能、吞咽功能、二级康复过渡至三级康复等三个方面进行系统性干预。经过两个月的治疗，患者的吞咽功能、肢体功能恢复良好，生活自理能力提高，康复出院后继续门诊三级康复治疗及居家延续护理，生活质量得到持续提升。

Abstract

This article summarizes the application experience of integrated traditional Chinese and Western medicine nursing intervention based on the 5E rehabilitation model in the secondary rehabilitation nursing of a patient with cerebral infarction. The 5E rehabilitation model is patient-centered and in line with the holistic view of traditional Chinese medicine. Through effective interventions in five aspects： encouragement， education， exercise， work， and evaluation， it systematically intervenes in three aspects： limb function， swallowing function， and the transition from secondary rehabilitation to tertiary rehabilitation. After two months of treatment， the patient's swallowing function and limb function have recovered well， and their ability to take care of themselves has improved. After rehabilitation and discharge， they continue to receive outpatient tertiary rehabilitation treatment and home-based continuing care， and their quality of life has been continuously improved.
